Job Description: 

Seeking an experienced Registered Nurse (RN) to join our dedicated home health team in Eureka, CA. The RN will deliver high-quality, patient-centered care to individuals in their homes across the assigned territory. The ideal candidate is compassionate, detail-oriented, and skilled in Oasis documentation and home health case management.

Roles and Responsibilities:

• Provide skilled nursing care to patients in their homes, ensuring individualized treatment plans are implemented and followed.
• Conduct comprehensive patient assessments, including Start of Care (SOC), Resumptions of Care (ROC), Recertifications, and Discharges (each worth two points).
• Complete routine follow-up and other skilled nursing visits (each worth one point) to meet productivity expectations of 6 points daily .
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care and ensure continuity of services.
• Educate patients and caregivers on disease processes, medications, and home health care procedures.
• Accurately document all patient interactions and updates using the HCHB (Homecare Homebase) EMR system .
• Adhere to all clinical and regulatory policies and procedures in compliance with federal, state, and agency standards.
• Demonstrate effective communication and time management skills while managing multiple patients and travel routes.

Education and Qualifications:

• Required: Active and unrestricted Registered Nurse (RN) license in the state of California.
• Experience: Minimum of 1 year of recent Home Health RN experience required.
• OASIS E1 experience required.
• EMR: Proficiency with HCHB (Homecare Homebase) required.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current BLS (Basic Life Support) certification from the American Heart Association.
