Job Description

We’re seeking a compassionate and detail-oriented Hospice Registered Nurse Case Manager to join our dedicated team serving patients and families throughout Lancaster and York Counties . This role offers an opportunity to provide meaningful, end-of-life care that focuses on comfort, dignity, and quality of life.

The Hospice RN Case Manager is responsible for coordinating all aspects of patient care, performing comprehensive assessments, and collaborating closely with the interdisciplinary hospice team to deliver individualized, compassionate care in the patient’s home or facility setting.

Schedule & Coverage
  • Shift: Monday–Friday, 8:00 AM – 4:30 PM
  • No weekends or on-call requirements
  • Territory: Lancaster and York Counties
  • Productivity: 5–7 visits per day
Responsibilities
  • Perform comprehensive nursing assessments, admissions, recertifications, routine, PRN, and death visits.
  • Develop and implement individualized hospice care plans in collaboration with physicians, social workers, and interdisciplinary team members.
  • Coordinate care transitions and ensure continuity of services across all settings.
  • Provide education and emotional support to patients and families regarding disease process, symptom management, and end-of-life care.
  • Monitor and document clinical progress, ensuring compliance with regulatory and organizational standards.
  • Identify opportunities for health promotion and illness prevention where appropriate.
  • Serve as an advocate for patients and families while protecting their privacy and dignity.
Qualifications
  • Active Registered Nurse (RN) license in the state of Pennsylvania (compact accepted)
  • BLS/CPR certification (required)
  • Valid driver’s license , reliable vehicle, and current auto insurance
  • Hospice experience required
  • HomeCare HomeBase (HCHB) proficiency required
  • Strong communication, assessment, and organizational skills
  • Associate or Bachelor’s degree in Nursing (BSN preferred)
