GENERAL SUMMARY : Under the direction of the Clinical Manager, Primary Care and/or health care provider, the Triage Nurse will provide direct clinical care and care management as assigned. Participate in Health Center and community activities to promote, advance or sustain CAH/P and MCHC goals and programs; this can include committee membership, presentations, or health related activities, etc. Supports the patient-centered medical home (PCMH) by working with patients, families, providers, and staff to promote timely access to needed care, providing daily continuity of care coordination, and education patients, families, and caregivers to understand patients care plan and self-care management responsibilities. The Triage Nurse works as a member of the care team. There is an ongoing interaction of team members to discuss roles, responsibilities, communication, and patient care coordination working together to provide and enhance the care provided by the practice. May assist with training and mentoring of clinical support staff or students as assigned.

ESSENTIAL JOB FUNCTIONS :
Clinical Functions
  • On a daily basis, triage calls from parent/guardian for patients with medical issues and determines the best course of action based on assessment of information provided and subsequently providing recommendations for home care, emergency care, or a doctors visit based on the severity and circumstances of their condition. Triage and recommendations to be completed according to standards of care and agency standards.
  • Handles return communication for clinical inquiries by phone from Child Protective Services (CPS) regarding inquiries of abuse or neglect of a child.
  • Assists with clinical tasks as needed as directed by the Clinical Manager, Primary Care. Including but not limited to professional nursing care including dispensing of medications, assisting with minor surgical procedures or other treatments as prescribed by the provider.
  • Develops and implements plan of care in conjunction with families and interdisciplinary team members using the Nursing Process.
  • Provides clinical education to patients and families based on identified needs.
  • Assists in identification and evaluation of social determinants of health and barriers to care.
  • Identifies community resources to assist clients and families with SDOHs and make referrals to other service agencies or interdisciplinary team members as appropriate.
  • Conduct nursing visits daily, ensuring timely and appropriate care.
  • Train and onboard newly hired nurses.
  • Assesses the needs of clients/families utilizing observations, examination/testing, and/or interview techniques.
  • Maintains program records according to agency, department, and program procedure.
Administrative Support
  • Participates in chart reviews and other quality assurance activities.
  • Utilizes computer skills to collect data, schedule patients, and access information needed for patient care.
OTHER RESPONSIBILITES :
  • Attends internal/external educational programs, reads professional journals and books to maintain or enhance skills/competencies.
  • May be asked to participate in MCHC hosted events and community outreach activities.
  • Participates in assigned meetings, trainings, and Child & Adolescent Health programs.
  • Follows established written agency policies and procedures in accordance with Standards of Care.
  • Other job-related duties as assigned.
QUALIFICATION STANDARDS :
  • Requires the completion of a bachelor's degree in nursing from an accredited college/university or graduation from an accredited program of Professional Nursing with an associate's degree.
  • Must have a current Michigan Registered Nurse license in good standing.
  • Current healthcare BLS certification required.
  • Requires a minimum of two years of progressively more responsible post-educational clinical work experience in pediatrics. (Pediatric primary care setting is preferred.)
  • Prefer 1-2 years of experience in outpatient pediatric and/or maternal/infant services, community health or nurse navigator experience.
  • Must be experienced in interdisciplinary team functioning, knowledge of community resources and understanding of the health/social needs of families.
  • Must have a high degree of communication and interpersonal skills to effectively interact with children, adolescents, parents, neighborhood groups, school staff, community agency personnel, administration, associates, and visitors.
  • Basic computer skills required: Microsoft Word, Excel, PowerPoint and Outlook.
  • Intermediate computer skills required: EHR/EMR
WORKING CONDITIONS :
  • Work is performed primarily in an office or clinical environment with no unusual discomfort due to temperature, dust, noise, or exposure to hazardous materials. Some job duties require home visitation. The home visits encompass the areas of Flint and Genesee County. Some of these areas may be associated with drug trafficking, prostitution, theft, or other violent crimes.
  • Occasionally the work sites may include areas that are unsanitary, unkempt, with risk of exposure to insects, vermin, rodents, unpleasant odors, or infectious diseases.
